Players take on the role of a cult leader, starting from the comfort of their home to build respect among followers. The game emphasizes human beings as the most valuable resource, with every character—from civilians to enemies—serving as a potential recruit.

Recruitment involves spreading propaganda, managing infamy, and making the cult appealing enough to join. Once followers are acquired, players can assign them tasks based on their strengths and weaknesses, such as farming for farmers, persuasion for smooth-talkers, or healing for doctors. Followers must be kept happy to prevent them from leaving, as a leader without followers is merely a 'lonely weirdo.'

The core of the experience lies in constructing a cult compound from the ground up, designing buildings, defenses, and managing resources while putting followers to work. Players craft unique belief systems to center their followers' lives around, with options ranging from UFOs and cannibalism to intermittent fasting or drum circles. These choices carry consequences, as locals, police, and the government may intervene to halt the cult's activities.

The game features a permadeath system, where deceased characters remain dead permanently, adding weight to decisions amid the apocalypse theme.
