Region Dalarna links death partly to journal system switch

A woman underwent an examination revealing an aneurysm on the aorta. The district doctor contacted a specialist physician, but her visit to the unit occurred two weeks later than planned. She died before the operation could be performed.

Region Dalarna's investigation shows the delay was partly due to the switch to the Cosmic journal system in autumn 2025. “The incident occurred during a period of unusually high workload in connection with the journal system change,” says Robert Blomgren, chief physician at Region Dalarna. The probe identifies unclear routines for urgent visits and documentation shortcomings as contributing factors.

The region states: “The incident is assessed to have been partly caused by Region Dalarna switching health information systems in October 2025, which has brought greater challenges than anticipated for healthcare staff.” Management is implementing training programs and system configuration changes to prevent recurrence.

Meanwhile, a group of doctors in Dalarna demands an external investigation and a disaster commission to terminate the agreement with the Cosmic provider. The system switch has been heavily criticized by local healthcare staff.
