Japan were without coach Eddie Jones, who was serving the last game of a four-match suspension. Forwards coach Neal Hatley took his place.
Flanker Ben Gunter, who scored one of Japan's three tries, said: "This is a big statement for Japan. Italy are a very strong team."
Uncapped university student Ryunosuke Ito started at fly-half and received praise from Italy coach Gonzalo Quesada, who called him "a player of the future."
