In Pigface, players assume the role of Exit, a character with a troubled past who awakens in a warehouse with an explosive device implanted in her skull. A mysterious voice on the phone instructs her: "If it feels like something's been implanted at the base of your skull, that's because there has. It's in your best interest to listen to me." Forced into cooperation, Exit undertakes combat sandbox-style missions as a gun-for-hire to earn money and upgrade her arsenal from the Black Market.
Key gameplay elements include purchasing weapons, armor, and equipment, as well as unlocking masks that grant various abilities to customize playstyles—whether charging in with bullets, using melee weapons like a rusty pipe, setting traps, or sneaking undetected.
