Fox expressed hesitation about Dr. Gregory House's Vicodin addiction but urged the creative team to handle it honestly if they proceeded.
Series creator David Shore recalled the network's single major note during development. "There was hesitation by the network about House being addicted to Vicodin, but that hesitation was a good hesitation," he told TV Guide. The network advised the team to treat the issue seriously rather than as a joke.
Shore confirmed the writers shared that view from the start. "We intended to go with this as a real issue, a real problem, a real dilemma for this character," he said. The character's chronic leg pain, stemming from tissue death in a quad muscle, provided the basis for the addiction.
Executive producer Katie Jacobs noted that House's vulnerability came through his pain rather than a desire for a high. "You call him a drug addict, and that's true, but he's popping all that Vicodin because he's in pain," she said.
