Region Dalarna is investigating whether a patient's death can be linked to issues in the Cosmic electronic health record system. The patient was not summoned for surgery on time and died while waiting. Dalarnas läkarförening attributes the incident to system flaws, as first reported by P4 Dalarna.
"There has generally been a lot of trouble with the summoning process in Cosmic," says Daniel Svedin, a member of the medical association. The event took place before year-end, a couple of months after Cosmic's implementation in the region.
SVT Dalarna has previously reported on similar issues with appointments and follow-ups in the system. The region emphasizes its patient safety efforts.
"We take all signals regarding patient safety very seriously. We follow up on all deviations reported, as part of our ongoing patient safety work," says Åsa Dedering, director of health and medical services at Region Dalarna.
