Uppsala receives honorable mention for elderly care collaboration

Uppsala Municipality and Region Uppsala have received an honorable mention at the Brobyggarpriset for the Elderly Care Unit. The unit brings together various healthcare professionals to provide home care for the elderly. The award praises collaboration across organizational boundaries with the patient's needs at the center.

The Elderly Care Unit is a joint initiative between Uppsala Municipality and Region Uppsala. It combines municipal and regional primary care to staff the operation and provide care in the patient's home. The aim is to create security, continuity, and involvement for the elderly.

"We are proud of the work we do together. This is proof that our joint approach makes a difference for the elderly receiving home care," says Magdalena Nyman, division manager for health and medical care in Uppsala Municipality.

The approach is part of the shift to proximity care and aligns with Uppsala Municipality's goal to be an age-friendly municipality. It has resulted in fewer acute care visits and readmissions. The jury highlights the sustained collaboration across principal boundaries.

"This is a clear example of how we in Uppsala develop care together across organizational boundaries, with the individual's needs as the starting point. The results show that it is possible to both increase security for the elderly and use our resources more efficiently," says Lenita Granlund, director of the care and elderly care administration.

The Brobyggarpriset is awarded by Dagens Medicin and Dagens Samhälle along with trade unions and professional organizations to break silo thinking in healthcare.
