Sweden investigates state takeover of forensic psychiatry

The government appoints an inquiry into full or partial state responsibility for forensic psychiatric care due to longstanding overcrowding. Social Minister Jakob Forssmed (KD) calls it the largest reform ever in the field.

Occupancy in forensic psychiatric clinics stands at 109 percent nationally according to the latest assessment. This has resulted in convicted individuals lacking places and either remaining in custody or moving freely in society.

Forssmed highlights security risks and displacement effects on ordinary psychiatry. The government has previously tasked the National Board of Health and Welfare and allocated funds to regions, but deems these steps insufficient.

The inquiry will examine how full or partial state responsibility can be implemented. SKR welcomes the review but wants regions to continue providing care. In 2025 more than 1 900 patients received care under the Forensic Psychiatric Care Act.
