Medical reasons for women to sleep without underwear

Sleeping without underwear is recommended to maintain women's intimate health. Nurse Hailey Pucket explains that this habit helps balance vaginal pH and prevents irritation. This simple practice allows the intimate area to breathe at night.

Sleeping without underwear may sound taboo, but it offers medical benefits for women's intimate health. Hailey Pucket, a Tennessee-based women's health specialist nurse and content creator, explains this in her Instagram post. She stresses that sleep habits affect vaginal health.

Hailey advises women to avoid underwear at night as it can make the intimate area moist and disrupt vaginal pH balance, leading to increased discharge. “There are many reasons why I recommend this, but the main one is that when you sleep, it's time for the vaginal area to ‘reset’,” she said, quoted from Hindustan Times on Thursday, November 13, 2025.

Further, Hailey notes that trapping moisture around the vagina and vulva with underwear disturbs pH equilibrium. “If the pH keeps changing, the body will produce more discharge to balance it again. Many misunderstand, thinking they need underwear because of discharge. Actually, the opposite—don't wear it,” she added.

For those uncomfortable sleeping bottomless, Hailey suggests loose sleepwear like boxer shorts to maintain airflow. Avoid tight clothing resembling underwear. According to Hailey, this habit can bring significant changes for women dealing with frequent discharge or other vaginal health issues.
